Job summaryThe Diabetes Specialist Practitioner will work autonomously across primary, community, urgent care, and secondary care settings, managing complex diabetes cases and providing education, support, and clinical advice to healthcare professionals, patients, and carers. The post holder will practice autonomously within their defined specialty to provide patient centered quality care. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice.
Main duties of the jobTo provide clinical leadership and expert specialist diabetes care within an admission avoidance and community-based service. The post holder will lead the development and delivery of evidence-based interventions aimed at reducing avoidable hospital admissions, supporting early discharges, and improving outcomes for people living with diabetes.

Job responsibilities- Provides specialist diabetes care and clinical leadership across community. primary care, urgent care, and secondary care interfaces.
- Manages a complex caseload of patients with diabetes who are at high risk of hospital admission or readmission.
- Delivers rapid assessment, intervention, and follow-up to support admission avoidance and early support discharge.
- Provides specialist advice and support to healthcare professionals across multiple care settings.
- Leads service development initiatives aimed at reducing diabetes-related admissions and improving patient outcomes.
- The post holder will practice autonomously within their defined specialty to provide patient centered quality care. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice.
- The post holder's job plans should deliver a minimum of 80% clinical component and 20% related to service management, education and research.
- Provide highly visible and accessible professional leadership and demonstrate expert knowledge and standards of clinical practice.
Person SpecificationKnowledge & ExperienceEssential- Significant demonstrable experience as a Diabetes Specialist Nurse
- Demonstrable experience managing complex diabetes cases
- Demonstrable experience working across primary and secondary care interfaces
- Demonstrable experience leading service improvement projects
- Demonstrable experience delivering education and training programmes
- Demonstrable experience in admission avoidance, urgent care, or community services
Desirable- Demonstrable experience in service redesign and transformation
- Demonstrable research and publication experience
QualificationsEssential- Registered Nurse (Level 1) with current registration with the Nursing and Midwifery Council
- Degree in Nursing or related healthcare discipline
- Postgraduate qualification in Diabetes Care or equivalent experience
- Independent Non-Medical Prescriber qualification
Desirable- Master's degree or working towards
- Advanced Clinical Practice qualification
