Job Purpose:
The Work and Health Employment Coach plays a key role within the WorkWell programme, supporting individuals with health-related barriers to gain, retain, or return to employment. Working as part of a multidisciplinary Primary Care Network (PCN) team, the post holder provides personalised coaching, health and employment advice, and practical support to help participants overcome challenges affecting their ability to work. The role focuses on early intervention, reducing health inequalities, improving wellbeing, and enabling individuals to achieve sustainable employment outcomes.

Key Responsibilities:

• Manage a caseload of participants experiencing physical health, mental health, social, or employment-related barriers.
• Carry out holistic assessments to identify barriers to employment and develop personalised action plans.
• Provide one-to-one coaching, mentoring, and motivational support to help participants achieve their employment goals.
• Deliver health and employment advice to support individuals to start, stay, and succeed in work.
• Receive and process referrals from Primary Care teams, Jobcentre Plus, employers, and partner organisations.
• Verify participant eligibility and ensure timely access to services.
• Coordinate referrals and signposting to specialist health, wellbeing, employment, and community support services.
• Build effective relationships with healthcare professionals, employers, voluntary organisations, and external stakeholders to support participant outcomes.
• Facilitate group sessions and workshops where appropriate to support cohorts of participants.
• Maintain accurate case records, action plans, and participant documentation using designated management information systems.
• Collect, monitor, and report service performance and participant outcome data.
• Promote awareness of the WorkWell programme and provide information, advice, and support to partner organisations and employers.
• Travel across the local area to meet participants and deliver services within community settings when required.
• Work collaboratively across the wider Work and Health Coach network and provide support to neighbouring PCNs where necessary.

The post holder will work closely with:

• GPs, Primary Care teams, and other healthcare professionals.
• Patients, carers, and community support services.
• Jobcentre Plus and Department for Work and Pensions representatives.
• Local authorities and voluntary sector organisations.
• Employers and employment support providers.
• WorkWell partners and wider Primary Care Network colleagues.

• Has experience of working in a health or employment coaching role, social prescriber or other similar role .

Desirable

• Has experience of working in, or closely with, Primary Care.
