Description

Job summary
We are seeking an experienced and innovative Band 7 Occupational Therapist to join our existing community Frailty Clinical Specialist Occupational Therapists working in our East PCN.

The post-holder will work as part of our multi-disciplinary PCN Wellbeing Team, which includes two other Frailty Occupational Therapists, Social Prescribers, a Health and Wellbeing Coach and Safeguarding Care Coordinators.

This role is an extended scope, advanced practice role working in the community with people in their own homes. The purpose of the role is to provide advanced Occupational Therapy clinical assessments, interventions and planning. This includes advanced care planning discussions. Liaising closely with the MDT including the GPs at the surgeries to facilitate and support these assessments. To proactively manage frailty and ageing well in the community, reducing un-necessary hospital admissions and supporting individuals to achieve the best possible quality of life.

Main duties of the job
To undertake comprehensive frailty assessments from an advanced clinical Occupational Therapy perspective. Delivering specialist Occupational Therapy interventions to support independence, function, safety and quality of life.

Deliver person centred, specialist Occupational Therapy intervention and goal planning, working collaboratively with the person, their support network, GPs and the wider MDT to improve patient outcomes.

Identifying risks, proactive planning and strategies to reduce unnecessary hospital admissions in collaboration with the wider MDT.

Facilitation and proactive engagement in Occupational Therapy practice placement education.

Person Specification
Education & Qualifications
Essential
  • Professional Qualification to degree level or equivalent in Occupational Therapy
  • HCPC Registration
  • Significant post graduate experience and CPD relevant to role / Specialist courses (or equivalent experience)
  • Evidence of or willingness to undertake further training at Masters level

Desirable
  • Leadership/Management qualification

Skills, Knowledge & Abilities
Essential
  • Broad and specialist knowledge of relevant patient conditions and therapy interventions
  • Computer Skills including word processing and e-mail
  • Demonstration of knowledge of core and highly specialist Therapy skills
  • Broad, developed and advanced knowledge of relevant conditions and therapy interventions
  • Extensive evidence of ongoing CPD in relevant area
  • Experience of multi-disciplinary communication, assessments and record keeping
  • Able to make independent, sound, advanced clinical decisions and problem solve
  • Able to manage and facilitate others in dealing with busy case load and prioritise
  • Ability to work autonomously using highly specialist knowledge, skills and initiative in sometimes complex situations acknowledging role boundaries

Experience
Essential
  • Extensive NHS or equivalent experience in specialist area e.g. community or community services
  • Experience of team leadership, management and positive role modelling
  • Student practice placement educator and experience of having students
  • Experience of using/fitting adaptive equipment
  • Supervisory experience

Other Factors
Essential
  • Ability to fulfil the travel requirements of post
  • Able to meet physical & moving and handling requirements of post. i.e. re-positioning of a patient, use of moving & handling equipment, approved manual physical movement practices of patient
  • Able to deal sensitively and safely with blood and bodily fluids
