We are inviting applications for sponsorship leading to a PG Dip and NMC recordable Specialist Practitioner Award (District Nursing). You must be able to demonstrate considerable experience in nursing, up to date extended clinical skills relevant to the service, leadership qualities, demonstrate delivery of evidence based practice and be passionate about your chosen field as well as share our Trusts commitments and values.
The programme involves taught modules at Birmingham City University and a clinical placement within the trust. The combination of clinical and academic content will enable you to deliver knowledge, clinical skills and leadership to support high quality, evidence based practice. You will focus on advanced history taking, physical examination and V300 independent and supplementary prescribing. You will also develop a robust understanding of complex case management and learn to use evidence to underpin outstanding delivery of care.
As a District Nurse, you will be passionate about leading a team of community nurses and support workers to deliver holistic nursing care to people in their won homes as part of an integrated locality team working with our MDT partners.
Main duties of the job- To work in a clinical nursing capacity within a community setting, primarily within a person's home.
- To assess, identify and record an individual's nursing needs and their ability to undertake Activities of Daily living in the home environment.
- To complete risk assessments/screening prior to implementing assessment and /or intervention.
- Undertake specialist nursing assessments to minimise clinical and non-clinical risk in the workplace and take appropriate action in line with local guidelines
- To support an ethos of health promotion and self-management for all patients and empower users and families to participate in the management of their illness or disability.
- To be responsible for ensuring appropriate assessment of care needs, and the development, implementation and evaluation of specialised programmes of care are undertaken
- To be able to evaluate the clinical effectiveness of your intervention and adjust clinical practice accordingly, in line with Organisation's Clinical Governance guidelines.
- Support teams to provide information to patients, their families and carers both formal and informal, so that they can self-manage their condition where possible and make informed choices about their care plan, current and future care needs, Prepare the patient and their family/carer for changes in the patient's condition and support choice for end of life care.

Person SpecificationEducation and qualificationsEssential- RN1
- Active NMC registration
- Evidence of continued professional development via post graduate course
- PS/PA competency
- Broad range of clinical nursing skills e.g venepuncture, syringe driver, catheter, compression therapy etc
ExperienceEssential- Minimum 2 years community nursing experience
- Good knowledge of management of long term conditions
