Job summary
Are you an ambitious District Nurse with a Specialist Practice Qualification (DNSPQ), or nearing completion of your programme, looking to take the next step in your career? An exciting opportunity has arisen to join our Community Nursing Neighbourhood Teams in Doncaster as a District Nurse. This is an opportunity to play a key role in delivering high-quality, person-centred care within patients' own homes, leading the coordination and management of complex caseloads whilst supporting individuals to remain independent for as long as possible. Our Community Nursing service is undergoing an exciting transformation through the development of Neighbourhood Teams, bringing together integrated community services to provide coordinated, proactive care closer to home. We are looking for compassionate, motivated and forward-thinking clinicians who can demonstrate excellent clinical knowledge, sound professional judgement and a passion for delivering outstanding community nursing care.

Main duties of the job
As a District Nurse, you will provide clinical leadership for your allocated caseload, coordinating the assessment, planning, delivery and evaluation of care for patients with complex nursing needs within their own homes and community settings. Using a case management approach, you will work in partnership with patients, families and multidisciplinary colleagues to promote independence, support self-management where appropriate, and help people remain safely in their preferred place of care. You will provide professional leadership and support to the wider community nursing team, acting as a role model for high standards of clinical practice. Working collaboratively across health and social care organisations, you will contribute to safe, effective and coordinated care delivery while supporting the ongoing development of neighbourhood-based services. The successful candidate will demonstrate excellent communication, assessment and decision-making skills, with the ability to prioritise a complex workload, manage risk and respond flexibly to changing patient needs. You will contribute to service improvement through clinical audit, quality improvement initiatives and the implementation of evidence-based practice. The role also includes supporting the learning and development of colleagues through supervision, coaching, mentorship and clinical leadership, helping to foster a positive learning culture and continual improvement across the service.

Qualifications
Essential
  • Registered Nurse (adult) with current NMC registration
  • Specialist Practitioner District Nurse qualification

Desirable
  • V300 prescribing qualification

Experience

  • Demonstrates the ability to work as a Specialist Practitioner District Nurse within the community.

Knowledge
Essential
  • Identification and evaluation of the best evidence-based practice in order to deliver effective health care intervention.
  • A clear understanding of government policy for the provision of health care delivery
  • To be able to work within the MDT and have a knowledge of wider service available.
  • Have an understanding of service improvement
  • Awareness of political policies and Government drivers - that may have an impact on local communities

Skills
