Job SummaryAn opportunity has arisen for a Nurse Practitioner to join our friendly team. We are looking for a highly motivated individual, ideally with experience of working in primary care to work alongside and support the GPs.
The successful candidate will see patients with minor illnesses and assist with chronic disease management as well as carrying out home visits.
Working hours are negotiable.

About UsWe are a friendly, well-organised 3 Partner Practice serving around 8,500 patients. We are also part of 5 practices that make up the North Easington Primary Care Network which has an excellent track record for collaboration, care and support.
Our Practice Team includes 5 GPs, Nurse Practitioner, 2 Pharmacists as well as a very experienced Nursing team and supportive administration team.
We have regular clinical meetings, practice meetings and MDT meetings as well as Protected Time in / time out sessions.
Support and mentorship will be provided and we actively encourage Specialist clinical interests and continued professional development.
Job ResponsibilitiesResponsible for attending to patients who wish to be seen by a health care professional; diagnosing and treating patients presenting with minor illness; seeing and advising people in respect of their continuing medical and nursing needs. The nurse practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
