Job summary
An exciting opportunity has arisen within our Primary Care Network (PCN) for an experienced and motivated Paramedic or Advanced Clinical Practitioner to work full or part time alongside our current Paramedic to deliver exceptional care to our patients in their own homes and within a primary care setting.

This post is suitable for a registered paramedic or an advanced clinical practitioner with an independent prescribing qualification or actively working towards or willing to gain this qualification within the first twelve months of recruitment.

Main duties of the job
  • Assessing and treating patients in their place of residence and those attending surgery.
  • Triage and treat patients, making any necessary referrals to other members of the primary health care team and MDT and/or organise admission to hospital if required.
  • Ensure clinical practice is safe and effective and remains within boundaries of competence and acknowledges practitioners own limitations
  • Advise patients on maintaining health and wellbeing, and refer to other team members as need arises.
  • Communicate at all levels within the multidisciplinary team ensuring effective service is delivered.
  • Maintain accurate, contemporaneous healthcare records appropriate to the consultation.
  • Ensure evidence-based care is delivered at the highest standards ensuring delivery of high-quality patient care.
  • Work within local policies and procedures.
  • Participate in the audit process, evaluation and implementing plans and practice change in order to meet patient need.
  • Contribute positively to the effectiveness and efficiency of the team and work colleagues.
  • Take responsibility for safeguarding children and vulnerable adults and work with other team members to ensure they are protected.

About us
Chester Central Primary Care Network (PCN) is a collaboration of six GP practices located in the centre of Chester, caring for a population of approximately 41,000 patients.

The practices are based in three locations, within walking distance of each other, four are based in the Fountains Health Building and we have the unique opportunity of the homeless practice within our PCN.

We are a friendly team with a number of additional roles (ARRS) including a wellbeing/social prescribing link worker, dieticians, GPs, clinical pharmacists, phlebotomist and many more who work alongside our paramedic team.

We are an inclusive PCN and have strong working relationships within the practices, ICB and external organisations.

Job description
The Paramedic/ACP will work as an autonomous advanced clinical practitioner providing holistic care for individuals including assessment, diagnosis, management, prescribing and treatment, to support primary care to deliver high quality patient services as part of a multi-disciplinary team (MDT) across our six GP practices. The post holder will assess, diagnose, treat and manage patients with a wide range of acute, chronic and complex conditions and include onward referrals and/or signposting patients to other allied professionals as required.

The post holder will use advanced clinical and communication skills to provide education to patients, promoting self-care, a health and wellbeing approach to care and empower patients to make informed choices about their treatment.

Care may need to be provided in other settings as the service develops.

Person Specification
Qualifications
Essential
  • Registered Paramedic with the Health and Care Professions Council (HCPC)
  • Degree in Paramedic Science (BSc or equivalent)
  • Evidence of continued professional development (CPD)
  • Demonstrable experience of autonomous clinical assessment and decision making
  • Eligibility to work in the UK
  • Willingness to undertake further training relevant to General Practice
  • Evidence of training in minor illness, long term condition management, or urgent care
  • Experience working within General Practice, Primary Care Network, or community healthcare
  • Independent Non-Medical Prescribing Qualification
  • Knowledge of primary and community healthcare
  • Advanced clinical practice, prescribing and examination skills
  • Excellent communication skills
  • Basic IT Skills
  • Evidence of commitment to own personal and professional development
  • Ability to manage and prioritise workload
  • Ability to work as part of an integrated multi-disciplinary team
  • Ability to work flexibly to meet the needs of the Primary Care Network
  • Understanding health education/promotion
  • Understanding of Clinical Governance and its significance and impact upon practice
  • Awareness of equality and valuing diversity principles
  • Personal Requirements
  • UK driving licence, with exclusive access to a vehicle

Desirable
  • Independent Prescribing qualification (V300) or demonstrable progress towards, or commitment to undertaking, the qualification
  • Postgraduate qualification in Primary Care / Advanced Clinical Practice (MSc, PGDip, or equivalent)
  • Mentorship or teaching qualification or experience supporting trainees
  • Knowledge/use of primary care IT software e.g. EMIS
