About us
Our Rapid Response team supports the District Nursing teams in Amber Valley and Erewash to provide a high quality service to our patients. We are a professional, friendly, and enthusiastic team who work closely with our colleagues to provide a holistic integrated service that enables and educates our patients to remain independent in their own homes, whenever possible, to prevent hospital admission and support a safe discharge home. This is increasingly important as care for patients become more community-focused. Our purpose in Derbyshire is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire. We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.
