Job summary
An exciting opportunity has arisen for a passionate Band 4 Therapy Assistant Practitioner to join our Falls Prevention service, working alongside our wider Rapid Response and Intermediate Care therapy services.

Our service is focused on helping people remain safe and independent within their own homes wherever possible. You'll play an important role in supporting patients following a fall, or those at risk of falling, by providing practical rehabilitation and therapy interventions that can help improve their mobility, confidence, safety and independence.

Working under the direction of our registered therapists, you'll undertake delegated assessments, deliver individualized rehabilitation programmes and monitor patients progress. You'll have the opportunity to work independently within your agreed competencies, while being supported by experienced Physiotherapists, Occupational Therapists and the wider multidisciplinary team.

This is a varied and rewarding community role where you can develop your therapy and rehabilitation skills while making a real difference to patients and their families.

Please note: a full UK driving license and access to a car are essential as travelling to patients within the community is a day-to-day requirement of this role.
