We are seeking Senior Triage Clinicians who have a passion for Urgent Community Response (UCR) to be based in the Worcestershire SPA based at Kings Court 2, Worcester. The post holder would be expected to work closely with the Neighbourhood Teams and system wide colleagues to triage referrals from professionals and facilitate appropriate care for patients and accessing alternative pathways to avoid unnecessary admissions/ambulance call outs.

You would be expected to work shifts across 07.30-18.00 with regular weekend and Bank Holiday working

This is an exciting and rapidly developing area of work with the aim of improving patient experience with particular focus for those at end of life as well as making positive impacts on system pressures, in particular to support colleagues in West Midlands Ambulance Service and in the A&E departments across the county.

This post could be filled by a Nurse or Physiotherapist also - these are being advertised under separate job reference numbers.

Job responsibilities
To triage UCR referrals that are received via the central UCR Hub with particular focus on Palliative Care and End of Life referrals. You will be responsible for triaging referrals for all 12x Neighbourhood Teams across Worcestershire, being able to make skilled judgement and quick and safe decisions as to suitability of referrals. You will then need to liaise with neighbourhood Teams regarding the management of the visits and individual NT demand and capacity escalating to operational UCR and SPA Lead as appropriate.

You will be expected to build strong relationships with relevant staff within the neighbourhood Teams as well as relevant stakeholders including Hospice, Specialist Palliative Care Team to enable current pathways to be mapped and strengthened with the aim of providing a standard response to partner organisations for those patients requiring end of life/palliative care in the community.

You will be expected to undertake daily calls with West Midlands Ambulance Service to identify and triage patients who may be suitable for NT management. As this is a new and developing service you will be expected to contribute to service development and be able to reflect and feedback constructively and manage rapidly changing situations.

To provide in-reach support and education into Worcestershire Royal Hospital.

You will be expected to work shift patterns across the operational hours of 07.30-18.30 including regular weekend and Bank Holiday working.

Qualifications
Essential

• Degree / diploma in Occupational Therapy
• Current registration with HCPC
• Evidence of relevant post graduate courses / Continual Professional Development maintained in portfolio

Experience
Essential

• A range of post graduate experience including work in a variety of physical and/or mental health settings / specialities Experience of working in a multidisciplinary / interdisciplinary team
• Recent experience in a community setting
• Experience of caseload management and prioritisation

Knowledge
Essential

• A range of evidence based Occupational Therapy clinical skills and knowledge appropriate to specialism
• Understanding of professional and ethical boundaries and code of conduct
• Knowledge of the roles and responsibilities of the professional groups within an integrated team

Skills & Abilities
Essential

• Ability to work proactively with other agencies to prevent hospital admission and facilitate early discharge
• Ability to make decisions autonomously, but liaise closely with and work across the multidisciplinary integrated team
• Ability to organise, prioritise and time manage own workload including use of effective delegation
