Job SummaryWe are seeking a highly motivated and experienced Practice Nurse with a recognised qualification in Diabetes Care to join our friendly and supportive primary care team. This is an exciting opportunity for a nurse who is passionate about delivering high-quality, patient-centred diabetes management and improving long-term health outcomes.
The successful candidate will play a key role in the delivery of diabetes services across the practice, working collaboratively with GPs, pharmacists, healthcare assistants, and the wider multidisciplinary team.
Main Duties of the Job- Lead and deliver comprehensive diabetes care clinics for patients with Type 1 and Type 2
- Undertake diabetes reviews, including annual assessments and care planning.
- Confidently initiate, titrate, and adjust insulin therapy in line with current clinical guidelines.
- Support patients with blood glucose monitoring, lifestyle changes, medication management, and self-management strategies.
- Interpret clinical results and develop appropriate treatment plans.
- Identify and proactively manage patients at risk of diabetes-related complications.
- Provide education and support to patients, carers, and clinical colleagues.
- Maintain accurate and timely clinical records using the practice's clinical system.
- Contribute to quality improvement initiatives and achievement of QOF and local enhanced service targets.
- Participate in chronic disease management and health promotion programmes as required.

Person SpecificationQualificationsEssential- Registered Nurse (NMC registered).
- Qualification in Diabetes Care (e.g., diploma, certificate, or equivalent recognised diabetes qualification).
- Previous experience working in Primary Care or General Practice.
- Demonstrable experience in managing patients with diabetes.
- Confident and competent in insulin initiation, adjustment, and ongoing management.
- Excellent clinical assessment and decision-making skills.
- Ability to work independently and as part of a multidisciplinary team.
- Strong communication and patient education skills.
- Competence in clinical systems and record keeping.
Desirable- Independent Nurse Prescriber qualification.
- Experience with continuous glucose monitoring (CGM) and diabetes technologies.
- Qualification in chronic disease management.
- Experience supporting patients with complex diabetes and multiple long-term conditions.
