An exciting opportunity has arisen for an experienced Specialty Doctor to join our dedicated Frailty and Older People's Medicine Service.

As part of our developing acute frailty model, you will play a vital role in delivering high-quality, patient-centred care for frail older adults. Working within a supportive, consultant-led multidisciplinary team, you will collaborate closely with consultants, resident doctors, nurses, therapists, and wider healthcare professionals to provide comprehensive assessment and management for patients across the Emergency Department (ED), Same Day Emergency Care (SDEC), and Care of the Elderly wards.

Main duties of the job
As part of the Acute Frailty Team, you will provide specialist frailty assessment and management within the Emergency Department (ED) and Same Day Emergency Care (SDEC). The role focuses on delivering rapid Comprehensive Geriatric Assessments, identifying appropriate alternatives to hospital admission, and supporting safe early discharge to improve patient outcomes and reduce unnecessary admissions and readmissions.

You will work collaboratively with multidisciplinary teams, community services, primary care clinicians, patients, and families to coordinate ongoing care and ensure continuity across healthcare settings. Responsibilities also include participating in multidisciplinary meetings, clinical governance, audit, teaching, quality improvement initiatives, and undertaking clinical procedures within your scope of practice.

In addition, you will support the education and development of resident doctors and medical students working on the Frailty ward.

Job responsibilities
Working as part of the Acute Frailty team, you will provide specialist frailty support across the Emergency Department (ED) and Same Day Emergency Care (SDEC), delivering rapid Comprehensive Geriatric Assessments to ensure patients receive timely, appropriate care while actively exploring alternatives to hospital admission where suitable. You will work closely with allied health professionals, community partners, patients, families, and primary care clinicians to facilitate safe early discharge, reduce avoidable readmissions, and support continuity of care. The role also involves collaborating with medical colleagues across a range of specialties, participating in multidisciplinary meetings, contributing to teaching, audit, governance, and quality improvement activities.

Additionally, you will actively participate in continuing professional development and postgraduate educational programmes to maintain and enhance your clinical expertise.

Education and Qualifications
Essential

• Full GMC registration

Desirable

• MD/PhD or equivalent
• MRCP or equivalent
• ALS

Experience & Knowledge

• Wide experience in Older Peoples Medicine and Acute Frailty
• Ability to work collaboratively with allied health care professionals in primary and secondary care

Desirable

• Knowledge of primary and community healthcare systems and IT

Academic Achievements

• Ability to use research outcomes in clinical practice
• Knowledge of using evidence base medicine to support decision making
• Experience of conducting clinical audit
