Job summary
South Kent Coast Community Mental Health Team is a diverse multi-cultural team that is looking to recruit a Band 6 Mental Health Nurse in a development post, to work within the team, providing specialist mental health care for people with multiple and complex needs that require intensive support.

You will carry out comprehensive, systematic nursing assessments which take account of relevant mental, physical, social, cultural, psychological, spiritual, and environmental factors, in partnership with the patient and others through interaction, observation and measurement.

Work collaboratively with other disciplines, patients and their carers to agree a holistic person centred care plan that addresses the needs identified through assessment.

To deliver and evaluate safe, competent patient centred care in partnership with the service user and their carers to support recovery.

Main duties of the job
  • To act as a Practice Assessor.adhere to the NMC registration and revalidation requirements.
  • To adhere to Kent and Medway Mental Health NHS Trusts policies, procedures and guidelines.
  • To support and behave in a way that upholds Kent and Medway Mental Health NHS Trustvalues.
  • To assess, act on and report any safeguarding concerns.
  • To optimise health and wellbeing; promoting self -care where possible, helping people to make choices about their needs and involve their carers where appropriate.
  • To safely manage, administer medications and monitor their effects.
  • To supervise a range of other staff and their activities as delegated.
  • To carry out specific duties relevant to the setting of community care medicines clinics, home visit depots.
  • Recording and reporting accurately any changes related to the patient's current behaviour and risks using redboard meetings daily or to team managers
  • Recording care given in a timely manner as per professional and local standards using approved Kent and Medway Mental Health NHS Trust systems.
  • Promoting a healthy, safe and secure environment by adhering to Trust statutory health and safety requirements and maintaining confidentiality of information at all times in compliance with Trust policies, procedures, protocols and guidance.
  • Acting as a role model and supporting new team members through demonstration and explanation of tasks and techniques.
  • Undertake and maintain all essential training to role.

The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Person Specification
Registered Mental Health
Essential
  • Current NMC registration
  • Knowledge and ability to apply relevant legislative frameworks and policies relating to care, e.g. Mental Capacity Act

Desirable
  • Previous experience of working in adult mental health
  • Previous experience of working within the NHS

Clinical knowledge
Essential
  • Sound understanding of secondary mental health diagnoses
  • Medication competencies

Desirable
  • Good understanding of Physical Health conditions

Essential
Essential
  • Good understanding of physical health conditions
  • Understanding of interventions offered in the community

Desirable
  • Previous experience of working in clozapine/depot clinics

Mental Health Knowledge
Essential
  • Understanding of the Mental Health Act 1983 amended 2007
  • Understanding of the Mental Capacity Act

  • Interest in delivering interventions
