Job Summary
We have an exciting opportunity to join our dynamic South West Specialist Palliative Care Team. This pivotal role focuses on delivering expert clinical care and compassionate support to patients with complex palliative and end of life needs across a variety of community settings. As a key member of the team you will: Lead clinical practice - Carry out holistic assessments, manage pain and symptom control and provide tailored emotional and psychological support to patients and their families. You will collaborate with GP practices, case managers, and other health and social care professionals. Drive evidence based practice - You will contribute towards and help implement best practice guidelines, undertake audit and share up to date knowledge across a wider health and social care community. Educate and Mentor - You will deliver training and education to colleagues in various health and social care settings and support the development of students and fellow professionals in palliative care. Weekend and Bank holiday working will be a requirement for the Trusted Assessor on the Single Point of Access Line. Main Duties of the Job

• To undertake comprehensive assessment of the health and psycho-social care needs of palliative care patients and plan appropriate care autonomously, reporting to Palliative and end of life care Clinical Nurse Specialist.
• To undertake pain and symptom management reviews of patients with complex needs as appropriate.
• To provide emotional and psychological support for patients and carers with complex needs.
• To undertake follow up and support visits in all community settings including residential/nursing homes, prisons and hostels as delegated.
• To demonstrate ability to manage a clinical caseload with supervision from the Clinical Nurse Specialist and take responsibility for overseeing all clinical caseloads in the absence of other team members working a weekend/bank holiday.
• Responding to the changing needs of patients (as they arise).
• Networking and liaising with GP practices, complex case managers, case managers and intervention leads and a variety of professionals across health and social care boundaries, statutory and voluntary organisations, patients and carers.
• Delivering educational programmes as the service requires.
• Providing advice and guidance as a Trusted Assessor on the Single Point of Access Line.

Job Responsibilities

• Work as an accountable and autonomous practitioner working alongside the community Clinical Nurse Specialists in the day to day running of the specialist palliative care service, working within a defined caseload.
• Provide specialist palliative care advice and support to patients and carers affected by a diagnosis of advanced cancer or other life-threatening long-term conditions, to address complex patient/carers needs in the promotion of quality of life and the experience of death.
• Provide specialist support and education on all aspects of palliative care to members of the multi-professional team in a variety of settings.
• Assess requirements for bereavement care and support for those carers and family members known to the service.
• To work in collaboration with Macmillan Cancer Support.
• Undertake shifts on the Single Point of Access line, to provide support and guidance for those with urgent palliative needs.
