We are looking for an enthusiastic Band 7 Community Nursing Team Lead to join our supportive and forward-thinking community nursing service.This is an exciting opportunity to play a key leadership role in delivering high-quality, compassionate care to patients within their own homes and communities. As a Team Lead, you will work closely with the Clinical Lead and District Nurse to ensure our service remains safe, effective, and truly person-centred.You will lead and support a dedicated team of community nursing professionals, promoting excellence in clinical practice while creating a positive and supportive team environment. Your leadership will help ensure smooth day-to-day service delivery, support staff development, and drive continuous improvement across the service.This role offers the chance to make a real difference -- not only to the patients and families we care for, but also to the staff you support and develop.Post holders will be available to work Monday to Sunday between 8am and 8pm. A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Main duties of the job
In this key leadership role, you will support services across localities to ensure the consistent delivery of safe, high-quality, and compassionate care within the community.Working closely with the Clinical Lead and District Nurse, you will lead and support a dedicated community nursing team, playing a central role in coordinating patient care, managing incidents, promoting staff development, and maintaining high professional standards. You will also contribute to clinical governance and service improvement, helping to ensure our service continues to evolve to meet the needs of the communities we serve.Your leadership will be visible and supportive, creating a positive team culture where staff feel valued, motivated, and empowered to deliver excellent care.This is a varied and rewarding role that combines 40% clinical practice with 60% leadership and management responsibilities, allowing you to remain hands-on in patient care while also driving improvements in service delivery, team development, and patient outcomes.

Job responsibilities

• To provide operational support across Guildford and Waverley Community Nursing, working with the District Nurses and Clinical Leads to manage staffing including return to work, sickness, formal reviews and appraisals.
• To manage off duty for Guildford and Waverley Community Nursing.
• To lead on Datix and investigations delegated from the Clinical Lead.
• To be clinical up to 40% of the role as required.
• To provide highly visible managerial leadership to the Community Nursing team and support of staff development in these areas.
• To ensure patient safety and clinical effectiveness, providing expert evidence based clinical advice and support to all staff within the locality.

The post holder will use an electronic patient held record system, EMIS as a record of care and all our nurses are provided with a work IPhone and IPad for mobile working.

Education and Qualifications
Essential

• Registered Level 1 Nurse with current Nursing and Midwifery Council registration.
• Assessor / NMC Supervisor / Mentorship

Desirable

• Completion of DN SPQ
• Clinical Supervisor

Knowledge, Skills and Experience
Essential

• Broad range of clinical skills relevant to community nursing, - including but not limited to Compression Bandaging, Dopplers, Venepuncture, Male & Suprapubic Catheter, Syringe drivers and Palliative care symptom management.
• Able to demonstrate experience of managing people, undertaking appraisals, investigations, formal and informal reviews, Supporting staff through HR processes.

Desirable

• Experience of undertaking audits, presenting feedback and owning action plans.
