Job summaryThe purpose of the post is to manage and provide specialist clinical practice for adult across both inpatient and outpatient areas. The CNS should be leading in the use of evidence based clinical practise in line with current research, guidelines and other available evidence. They should ensure that patients and their families are fully informed about their journey through the service (dependant on speciality). Utilise other members of the MDT when required to increase knowledge and skills to ensure delivery of reliable high-quality care.
Demonstrate comprehensive holistic assessment of patients utilising a variety of advanced clinical skills, while acting as a role model, in line with your Clinical Nurse Specialist role for your service.
Main duties of the job- Ensure that patients receive a high level of quality harm free care at all times and contribute to the monitoring of this through audit or other KPIs/ or outcome measures
- Manage your service within the Royal National Orthopaedic Hospital, ensure effective patient centred care is provided to patients who have or are at risk of complications related to your speciality
- Utilises advanced Nursing knowledge and skills to enhance the holistic management, experience and pathways for patients.
- To undertake independent research/ analyse audits on your service and act upon these (including action planning and identifying learning needs)
- Liaise with the Head of Nursing/B8/DDoN to advise and contribute to clinical governance activities within the trust around your speciality

Job responsibilitiesRotation between Bolsover Street and Stanmore sites when applicable to your banding
Clinical- As your service requires Autonomously seeing both Inpatients and Outpatients and seek support when appropriate. This may warrant undertaking clinical shifts to develop own practice and understanding of current issues.
- To work within the professional registration of the post holder acting as a role model through the provision of professional leadership and demonstration of competent practice. In extraordinary circumstances as defined by the Head of Nursing and in conjunction with the Deputy director of nursing- work as a member of the ward/centre/unit team to provide fundamental and safe levels of care.
- Ensure that you take the Lead role within the MDT in coordination of the patient pathway (and outpatients as required).
- Demonstrate in-depth knowledge of your service that is based on evidence and research to assess, plan, implement and evaluate episodes of care for individual patients.
- To provide expert knowledge, advice and clinical support to members of the multidisciplinary team, and ensure that all nursing and medical needs are understood and carried out in accordance with the present standards and protocols.
- Develop and co-ordinate Integrated Care Pathways (ICPs) of care to promote reduction of current waiting times, reducing delayed discharges and preventing re-admission/failed discharges into hospital.
- To assist in the development and implementation of local practice guidelines and protocols.
- To ensure that all information and knowledge provided is based on best practice and strives to meet the Trusts Values, vision, aims and objectives and should incorporate local and national policies, procedures, NICE guidelines and any other relevant legislation or national/international guidelines.
- Be available to offer advice on all aspects of your speciality, providing clinical advice and supporting Trust staff in meeting the standards and protocols in delivery of care for your speciality. Where appropriate give advice and support on your speciality to staff outside of the Trust
- Influence effective multidisciplinary teamwork and communication between health, social and voluntary organisations to deliver co-ordinated, seamless care.
- Co-ordinate/manage any relevant clinics for your service or/and be prepared to develop such clinics to benefit your patient group
- Independently discharge patients from clinics within agreed protocols and follow up (as per service requirement).
- Work autonomously, providing an effective nurse led service as needs dictate
- Organise blood tests, x-rays, and other appropriate investigations for your speciality (dependant on your skill level and training). Interpret the investigation results (ensure red flags are noted) and make appropriate plan of care, liaising with consultant of the speciality/GP, as required, regarding future treatment and management dependant on your speciality.
- Provide an effective and accessible telephone advice service for patients and families and their carers.
- Ensure patient records are maintained in an accurate, legible, complete and timely manner.
- Ensure that robust systems are in place for effective monitoring of performance against national and local indicators and any other reporting requirements. The post holder will be expected to be familiar with all local and NICE guidance relevant to your speciality
- As mentioned above, undertake the relevant courses to be a registered nurse prescriber for the medicines management of your speciality as determined by service need and in consultation with the non-medical prescribing and patient group directions group and to act as a resource for clinicians in relation to treatment and care for patients with your speciality needs.
