Main DutiesDiagnosing and treatment, a wide variety of illness and coordinating ongoing medical care. Prescriptions, referrals, interpreting clinical test and referring patients to appropriate service. Providing patients with health education and long term care. Conducting patient consultations (in person, telephone and home visits to assess diagnose and treat both acute and long term conditions
About UsWe are a very friendly well run practice with limited staff turnover. You will have the option to join the NHS pension scheme, 6 weeks paid leave and 1 week study leave over a 12 month period. Access to car park. We have a team of 3 GPs, 1 Paramedic Practitioner, 2 Nurses, 1 HCA, 1 Mental Health Practitioner, 2 Clinical Pharmacists. All external documents outsourced.
Job Responsibilities- Patient Consultations: Assess, diagnose, and treat acute and chronic illnesses via in-person clinic appointments, phone or online triage.
- Preventative Care & Screening: Advise on lifestyle changes, provide health education.
- Chronic Disease Management: Monitor and design ongoing care plans for long term health conditions such as diabetes, asthma, COPD, hypertension, Heart Disease.
- Care Coordination: Make professional referrals to secondary care specialist and collaborate MDT meetings.
- Administration & Compliance: Maintain accurate patient records, sign prescriptions and handle clinical autonomous decisions.
- Organisation: Time-management to efficiently handle paperwork and short 10 minute appointment slots.
Person SpecificationQualifications- GMC registration
- Must have experience in General Practice
