We are seeking an experienced Clinical Pharmacist to join our established clinical team and help shape the delivery of safe, effective and patient-centred prescribing services.
The Role
As a Clinical Pharmacist, you will play a key role in supporting the management of long-term conditions, medication optimisation, structured medication reviews and medicines reconciliation. You will work closely with GPs, nurses, care coordinators and other healthcare professionals to improve prescribing quality and patient safety.
This is an exciting opportunity for a Pharmacist who enjoys autonomous clinical practice, running independent clinics, making complex prescribing decisions and contributing to medicines optimisation, QOF and service development within a busy primary care environment.
Hybrid working can be considered for the right candidate, although the role is primarily practice-based.

Main duties of the job- Run independent long-term condition clinics eg; hypertension, diabetes, cardiovascular disease, asthma/COPD and other agreed areas.
- Assess, diagnose within scope, prescribe and manage patients independently.
- Manage medication authorisations, repeat prescribing and complex prescribing queries.
- Support QOF, IIF and Enhanced Service delivery.
- Lead medicines optimisation, prescribing safety and high-risk drug monitoring.
- Undertake clinical decision-making for complex patients in a busy primary care environment.
- Use population health management and risk stratification to identify patients for proactive review.
- Support care home, frailty and anticipatory care.
- Lead audits, quality improvement and prescribing projects.
- Provide supervision and mentoring to the wider prescriptions/ pharmacy tech team

Job responsibilitiesThe post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians in general practice.
The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy and produce recommendations for senior clinical pharmacist, nurses and/or GP on prescribing and monitoring.
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice.
The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
The post holder will be supported to develop their role to become a nonmedical prescriber.
Person SpecificationQualificationsEssential- Registered Pharmacist with the General Pharmaceutical Council (GPhC).
- Independent Prescriber qualification (or working towards).
Desirable- Primary Care Pharmacy Education Pathway (PCPEP) completed or in progress.
ExperienceEssential- Experience working in Primary Care, General Practice or a Primary Care Network.
- Strong clinical knowledge and understanding of medicines optimisation.
- Excellent communication and consultation skills.
- Ability to work autonomously and make clinical decisions.
- Commitment to continuous professional development.
Desirable- Primary Care Pharmacy Education Pathway (PCPEP) completed or in progress.
- Experience using Systmone.
- Experience of managing long-term conditions clinics.
- Experience leading prescribing audits and quality improvement projects.
