There will be opportunity for development of acute skills work across the renal and medical HDU with close liaison with ITU; invaluable experience applicable across all medical specialties

You will gain the opportunity to train in invasive procedures in a supportive environment with hands on consultant supervision and training.

Recent resident doctors coming through this post in recent years have gone onto permanent contracts within the trust or obtaining NTNs in their chosen specialty (often in renal)

Main duties of the job

• Provision of junior support to the Specialist Medicine and Acute Medicine services
• To provide daily care and management of general internal medical patients on wards 407 and 409 in Royal Derby Hospital
• Participation in out-patient renal clinics (Royal Derby and Queens Hospital Burton)
• Provide (with support and training) daily care to dialysis patients on haemodialysis and peritoneal dialysis
• Provide daily care and management in the Acute Renal Assessment Unit with consultant of the day supervision at all times
• Provide Acute renal referral cover in Derby and Burton for AKI and other specialist renal problems in conjunction with the referrals consultant of the week
• Active participation in Clinical Governance, audit and research activities.Keeping up to date and participating in relevant Continuing Medical Education and Continuing Professional Development (CPD)
• Participation in annual appraisals
• Provision of junior support to the Acute & Specialist Medicine services.
• Shared role in delivery of teaching & training to junior doctors & non-medical staff.

Job responsibilities
We are very pleased to be appointing a Senior Clinical Fellow (ST4+ equivalent) to work in Specialist Medicine as part of our team in renal medicine. This is a full-time, fixed-term service appointment for 12 months. Applications for less than full-time working patterns (LTFT) are very welcome.

The post is based at Royal Derby Hospital, the appointees will undertake duties within the renal unit covering both Derby and outreach support to Burton.

The post will involve 12 months of specialist medicine with an out of hours commitment to the renal rota which covers acute high dependency unit (HDU) admissions to renal as well as general medical HDU. This will be a 1:8 weekday evening cover and 1:8 weekend cover (no night shifts). Full resident. Renal Consultant available at all hours for support.

Senior Clinical Fellows (IM3 equivalent) work Monday to Friday, rostered between 08.45-17.15. As part of a ward multi-disciplinary team, the SCF will review patients daily and support consultant ward rounds to enable efficient senior decision-making and effective management of patients on their respective wards. In addition, there are daily board rounds with the opportunity to discuss the management of the ward patients with the consultant of the week.

During your inpatient work you will be based within the Department of renal medicine and work with our team of consultants, advanced care practitioners (ACPs), therapists and Discharge Support Officers (DSOs) to deliver a responsive service to medical patients throughout the hospital. Your speciality team will give you consistent support and further learning opportunities.

You will be expected to work as middle-grade doctors, seeing renal speciality referrals, medical High dependency referrals and leading your junior team with full consultant support. There is a consultant working on the renal inpatient ward and a separate consultant covering referrals with limited other commitments during their ward / referral weeks so you will be guaranteed full support when seeing complex referrals including plenty of opportunities for development and training.

You will also work on the dialysis unit and in the renal assessment unit (RAU) allowing you to gain supported experience and training in management of more specialised renal problems including acute glomerulonephritis and commonly encountered dialysis related complications and emergencies in our interesting multi-morbid population. We run Multidisciplinary Simulation sessions on the dialysis unit to aid your confidence in managing these areas.

The post can be tailored to individual requirements but centres upon the provision of care to the renal patients. There is scope to access opportunities that enable you to fulfil the alternative certificate required for stage 2 training. For instance you will be invited to all the regional registrar teaching days in renal and general medicine on a rotational basis with the other middle grades. Local renal training includes a weekly educational meeting, teaching outpatient clinics in renal modalities, fortnightly histology meetings, multidisciplinary radiology meetings, Acute Kidney Injury (AKI) service across two sites with support of AKI nurses as well as consultants to mention a few.

• MBBS or equivalent
• MRCP Part 1 or equivalent higher qualification
• Life support qualifications (ALS)
• Full GMC registration with a license to practice at the time of application
• Minimum of 6 months experience as a doctor in the NHS in medical specialties within the past 2 years

• Additional higher medical/scientific degree relevant to the specialty
