Main duties of the job- Triaging 2WW referrals
- Supporting the delivery of minor ops by providing pre assessment and supporting the admission and discharge process.
- Undertake autonomous clinical follow up with complex patients and facts which require analysis and interpretation of post-procedure patients both face to face and over the phone
- Providing patient education and building relationships with patients and relatives

Key responsibilities- To provide and receive highly complex sensitive information through specialist nursing care, education and advice to patients, their carers and healthcare professionals, for stone disease services through formulation of treatment pathways and conservative management that meet the specific needs of individual patients
- To utilise highly developed specialist knowledge, underpinned by theory and experience relevant to own practice to deliver a range of job responsibilities in a safe, effective, competent and confident manner, across the range of clinical nurse specialist policies, procedures and practices within the field of stones disease and other urological conditions
- Make sound decisions, which are ethically based in the interests of the patients and clients in the absence of protocols
- Act under indirect supervision of the Clinical Lead using current guidelines to plan and implement individualised treatment.
- Manages own caseload of patients whilst working as part of the Urological Multidisciplinary team, liaising with general practitioners where necessary to obtain or provide patient information relevant to their care and treatment
- Provide patient assessments, organising appropriate diagnostic tests, different modalities of imaging and investigations, and interpretation of these for treatment for stone disease in a timely manner in a nurse led clinic
- Triage 2WW referrals.
- Run independent clinics as appropriate.
- Support Minor Ops pathway by pre assessing, admitting and discharging patients.
- Uses highly developed skills to undertake physical examination in the provision of assessment, implementation and evaluation of patient care
Person SpecificationKnowledge and ExperienceEssential- Knowledge of relevant NICE guidelines
- Demonstrable experience as an independent practitioner and proven ability to work independently and run nurse led clinics
- Demonstrable experience in verbal, written and interpersonal communication skills
- Proven ability to communicate patient information and education
- Demonstrable experiencing in planning and delivering education sessions for both patients and staff
- Evidence of continual professional development
Desirable- Demonstrable experience of pre assessment
- Demonstrable experience in urology
QualificationsEssential- Registered Nurse/healthcare degree
- Valid NMC/HCPC registration
- Communication/counselling skills
- Knowledge of relevant NHS guidance and trends in specialist area
