This is a unique opportunity to play a pivotal role in the development and delivery of an innovative out-of-hospice specialist palliative care service, working collaboratively across Wakefield Hospice and The Prince of Wales Hospice.
Main duties of the jobYou'll combine hands-on specialist clinical care with leadership, service development and partnership working helping patients avoid unnecessary hospital admissions and supporting more people to receive compassionate care in the place they choose. As the SPCC, you will provide expert clinical leadership while helping to design and embed a new model of care that delivers timely, coordinated and person-centred support for patients and families across the Wakefield district. You will work closely with colleagues, community partners and the wider multidisciplinary team to improve access to specialist palliative care, reduce avoidable hospital admissions where appropriate, and enable more people to receive care in the place they choose.
About usWakefield Hospice, is committed to providing the highest level of symptom management and care for people who have advanced active, progressive and life threatening illness. Wakefield Hospice is a purpose-built specialist palliative care unit that opened in April 1990. Currently undergoing refurbishment to provide 16 single ensuite bedrooms, as part of the Fit for the Future project. The hospice also has a dementia care service, bereavement support and education services. Over the years the unit developed initially into a day therapy unit and ultimately into a drop-In centre providing therapies, information and care for patients, carers and bereaved families.
