Job summary
Willow Wood Surgery is seeking an experienced, knowledgeable and confident Clinical Pharmacist to join our friendly and supportive practice team. This is an exciting opportunity for a pharmacist to use their clinical expertise to make a meaningful contribution to patient care and medicines optimisation within primary care.

The successful candidate will work autonomously, managing their own workload and providing expert advice on medicines, prescribing and patient care. Key aspects of the role include undertaking structured medication reviews, supporting patients with polypharmacy and long-term conditions, managing routine repeat prescriptions, responding to medicines-related queries and promoting safe, effective and cost-effective prescribing.

We are open to applications from Band 6 pharmacists upwards, depending on experience and skills. Previous experience in primary care, a GP practice or PCN setting is desirable but not essential for the right candidate.

The role is offered at 22 hours per week on a permanent basis, with working days and hours negotiable. You will be supported by our GPs, who are available for advice when required, while giving you the opportunity to work independently and develop your role within a welcoming primary care environment.

Main duties of the job
The successful candidate will undertake structured medication reviews for patients, including those with polypharmacy and long-term conditions, and will review, authorise and manage routine repeat prescriptions in line with practice procedures and prescribing guidance. They will support medicines optimisation and prescribing safety across the practice, identify and manage medication-related risks, and ensure appropriate monitoring is undertaken. The successful candidate will respond to medicines-related queries from GPs, nurses and other members of the practice team, providing clinical advice and support regarding medicines and treatment options. They will work collaboratively with the wider multidisciplinary team to improve patient care and outcomes, support safe, effective and cost-effective prescribing, maintain accurate clinical records and work in accordance with professional and practice standards. The role will also contribute to the ongoing development of medicines management and optimisation within the practice.

We are open to applications from Band 6 pharmacists upwards, depending on experience and skills.

Although the role involves autonomous working, our GPs will always be available for support and advice when required.

• Registration with the General Pharmaceutical Council (GPhC)
• Right to work in the UK
• Significant post-registration clinical experience, ideally in primary care
• Able to work independently and manage own workload

Desirable

• Experience in a GP practice or PCN setting
• Long-term condition management qualification
