Job summary
This post is a Band 7 Enhanced Nurse Practitioner within the Rapid Response Nursing Team in Gateshead.

To work effectively within a multi-disciplinary team to manage patients identified as requiring an urgent community response to prevent a deterioration in their health and wellbeing. In addition, there will be planned activity to prevent avoidable admission to hospital and facilitate early supported discharge, where clinically appropriate.

This post is a senior autonomous clinician who will ensure the fundamentals of care are provided at the point of contact and will ensure delivery of a range of appropriate interventions to be able to meet the needs of the patients, within the community setting. Patients accessing the service will have complex needs and potentially numerous co-morbidities including frailty.

The autonomous practitioner will provide comprehensive healthcare sensitive to the needs of the population, responsible for senior clinical decisions, including advanced clinical assessments, history taking, diagnosing, planning treatment, prescribing and evaluation of care.

Working as part of a multidisciplinary team and across the primary, secondary and social care interfaces in Gateshead and will be responsible for coordinating and meeting the health care needs of people at risk of hospitalisation or requiring a more intensive level of care in the unscheduled, intermediate, palliative and EOL care environment.

Main duties of the job
  • Ensure the right care, is delivered at the right time, by the right person.

  • Support the Rapid Response Team lead to deliver high quality, patient centred care to Gateshead residents.
  • Support the team lead to manage the service on a day to day basis.
  • Act as/support the senior clinical decision maker for the team's Single Point of Access.
  • Support the team to deliver an urgent 2 hour community response with an emphasis of ensuring patient safety through effective prioritisation.
  • Support the team to deliver out of hours palliative care.
  • Support the team to deliver intravenous antibiotics at home.

Job responsibilities
  • To participate in the planning, development and evaluation of clinical services using advanced clinical skills and knowledge.
  • To provide specialist advice on the assessment, care planning and interventions of patients needs.
  • To support the point of contact ensuring patients receive care at the right time, right place by staff with the right skills.
  • To further develop knowledge and skills specific to the community setting through undertaking regular audit, research and development activities.
  • The post holder will have a community base but will also spend time linking to the Locality Teams, Primary and Secondary Care, care homes (nursing and residential) Gatdoc and NEAS.
  • A key part of the role will be cross agency liaison, training, research and development, and being a clinical resource for the local Health Community.
  • Clinical responsibility for a defined group of patients who access the services with accountability for the health outcomes for the individuals within their care on a session by session basis
  • To work with the accountable clinicians in the development, delivery and provider of complex care within the community setting
  • To ensure safe systems and processes are in place to support a multidisciplinary team working approach
  • To ensure all professional standards and Codes of Conduct are adhered to.
  • Facilitate the development of clinical expertise and advanced practice within the team to support the right person, right place right time agenda
  • To ensure patient referrals are triaged and allocated to the appropriate member of team, depending on clinical need.
  • Risk Stratification is appropriately managed in the best interests of patient and staff safety.
  • To provide senior leadership and clinical expertise in emergency and unscheduled care for a variety of multidisciplinary staff
  • Be responsible for, provide and monitor the standard of assessment, planning, implementation and evaluation of care ensuring the quality of care provided is of a high standard.
  • Act as a rapid response practitioner to oversee all aspects of clinical care planning and delivery to ensure maximum positive outcomes for patients and delegate to other members of the team as appropriate
  • Work as an autonomous practitioner, clinically, professionally and practically
  • To lead clinically and professionally ensuring compliance with all statutory and mandatory professional requirements
  • Continually develop own clinical expertise, knowledge and clinical skills to enhance effective care and support. Ensure this expertise is shared with other clinical staff.
  • Champion the development of leadership competencies within the team
  • To ensure continuity and co-ordination of care across the health and social care community
  • To develop protocols relating to advanced practice and work in an extended clinical role.
  • To continually review patient/client dependency, staff skill mix, patient management and shift patterns in line with patient/client needs and staff development across the 24/7 period

Person Specification

Qualifications

Essential
  • Diploma/Degree in Adult Nursing
  • Clinical skills/physical assessment skills qualification
  • Non medical prescribing/willingness to work towards
  • Valid UK driving license

Skills

Essential
  • Able to administer intravenous antibiotics and safely use central venous access devices
  • Provide palliative/end of life care to patients
  • Leadership skills

Experience

Essential
  • Significant post registration experience

Desirable
  • Experience/awareness of urgent community response agenda
