Job summary
The Band 6 Complex Care Nurse supports the implementation of the National Framework for Continuing NHS Healthcare (CHC) across Powys. The role involves completing in-depth nursing assessments, contributing to personalised long-term care planning, and coordinating eligibility processes for CHC and NHS Funded Nursing Care. The post holder case-manages individuals with complex needs, works within multidisciplinary teams, provides specialist advice and training, and helps ensure seamless, integrated care through collaborative working across health, social care, and partner agencies.

Main duties of the job
Clinical Responsibilities
  • Complete comprehensive nursing assessments and develop person-centred care plans.
  • Co-ordinate CHC eligibility assessments and undertake NHS Funded Nursing Care reviews.
  • Case-manage NHS-funded individuals and ensure ongoing review and integrated MDT working.

Communication & Engagement
  • Clearly communicate assessment outcomes to patients or legal representatives in writing.
  • Promote patient and carer involvement, ensuring consent and capacity processes are followed.

Planning, Governance & Quality
  • Contribute to the development and improvement of CHC pathways and assessment tools.
  • Participate in audits, risk management, peer reviews, and performance monitoring.

Training, Support & Multi-Agency Working
  • Provide training on NHS funding processes to multidisciplinary teams.
  • Act as a resource for colleagues across health, social care, voluntary and independent sectors.
  • Support safeguarding processes and represent PTHB at relevant meetings.

Management & Leadership
  • Act as a change agent supporting integrated care models.
  • Prioritise and manage own workload autonomously.

Digital, Information & Professional Requirements
  • Maintain accurate documentation and uphold professional standards, training, and development.

Qualifications & Knowledge
Essential
  • Registered Nurse - NMC registration
  • Postgraduate in Nursing or health-related subject or equivalent experience
  • Knowledge of current NHS care system issues

Experience
Essential
  • Clinical assessment and proactive caseload management
  • Manage patients with complex needs
  • Experience working across care boundaries and engaging in challenging negotiations
  • Motivate and promote innovation within teams
  • Working in complex situations with multiple partners
  • Proven ability to work under time pressures

Skills & Attributes
Essential
  • Highly competent in IT systems (email, word processing, spreadsheets)
  • Significant clinical experience assessing, planning, implementing and evaluating care for complex caseloads
  • Strong written and verbal communication skills
  • Effective multi-agency team working skills, including engagement with carers
  • Ability to write and present reports

Desirable
  • Some ability to speak, read and/or write Welsh, or an eagerness to learn

  • Ability to travel across the geographical area
