Job summaryWe are seeking an enthusiastic GP to join our established team at SLC Medical Group. As well as the larger towns the region has thriving market towns, picturesque villages, beaches, rolling hills, hidden valleys, and towns and villages that are not only steeped in history but offer affordable housing, outstanding schools and excellent transport links. The Lincolnshire Wolds is an area of outstanding natural beauty and is within easy reach of historic counties such as Derbyshire and Yorkshire. We have excellent road and rail links and some of the UKs most diverse and vibrant cities are easily accessible including Sheffield, Leeds and York.

Main duties of the jobThe post-holder will undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing off repeat prescriptions and dealing with queries, paperwork and correspondence in an appropriate timescale.
About usWe are successful, high achieving, PMS practices with a list size of 24,500 patients. Our 3 practice sites are located in purpose built Primary Care buildings. Our priority is to provide high quality patient-centred care, where personal development and special interests are encouraged. There are opportunities to be involved in areas such as GPST training, FY2 training, a wide range of enhanced services, palliative care or the local ICB.
Job responsibilities- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
- Screening patients for disease risk factors and early signs of illness.
- In consultation with patients and in line with current practice disease management protocols, developing care plans for health.
- Providing counselling and health education.
- Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate.
- Recording clear and contemporaneous consultation notes to agreed standards.
- Collecting data for audit purposes.
- Compiling and issuing computer-generated acute and repeat prescriptions.
- Prescribing in accordance with the Northern Lincolnshire Area Prescribing Formulary.
- In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
